About Us
Kaiser Permanente Southern California Home Care Services provides Home Health, Hospice and Home-Based Palliative Care services for all clinically eligible members in the Southern California Region.
Care Delivery is organized through seven licensed agencies and three branch offices spread from Antelope Valley to San Diego, between Los Angeles and Riverside. Our services are provided by a robust team of Registered Nurses, Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, Chaplains, Certified Home Health Aides and Administrative support staff committed to supporting all patients in our care.
Our agencies are licensed by the California Department of Public Health (CDPH), certified by Medicare and are accredited by the Joint Commission.
